Updating personal details overview

It is important to let us know if you change any of your personal details.

This includes:

• Name
• Address
• Telephone number
• Mobile number
• Email address (check the data security issues of letting patients change their email address via a personal submission and not via an app)

Changing your contact details

To update details like your name, address, phone numbers or email address:

• fill out a health form via Paco on our website 15 Sefton Road
• visit the surgery to fill in a change of details form from 08.00am to 6.30pm

Change of address

If you have moved to a new address, you will need to be in our catchment area to stay registered with our surgery.

Changing your date of birth or name

You will need to show us proof that you have changed your name or date of birth. This could be a marriage or civil partnership certificate, deed poll or birth certificate.
